A few things to have ready.

Use this checklist for your own notes. You do not need to upload personal documents here.

Bring what you already have.

  • Your most important questions.
  • Your current coverage details and any letters you received.
  • Names of doctors, specialists, and preferred hospitals.
  • A list of prescriptions, doses, and pharmacies.
  • Any employer coverage end date or retirement paperwork.
  • Questions about a spouse or dependent’s coverage.
  • Information about VA, TRICARE, or other benefits you use.
  • Notes about travel or living in more than one state.

Before you finish the conversation

  • Do I understand what to check next?
  • Which dates apply to my situation?
  • What information still needs confirmation?
  • Who should I call if I have another question?
