An ongoing point of contact
An agent-of-record letter tells SelectHealth which agent you want to represent you for the coverage identified in the letter. The form replaces a previous agent or agency appointment for that coverage and remains in effect until revoked or replaced in writing.
Read the actual SelectHealth letter before signing. Asking a question, booking an appointment, or requesting optional updates does not itself appoint an agent.
Meet with Health Plan Assistants at 2736 E Red Cliffs Dr, Suite 2, St. George, UT 84790, or schedule a phone consultation. Tell us which type of coverage you have so we can prepare for your appointment.
No. This is Health Plan Assistants, an independent insurance agency. For your member account, claims, billing, or an urgent coverage issue, use SelectHealth’s official member resources or the number on your ID card.
The AOR letter appoints an agent for the coverage identified in the letter. It is not a new plan enrollment application. Review any separate enrollment or policy-change request before authorizing it.
You can call or schedule a consultation to ask questions first. We will explain any authorization needed before helping with a specific policy.
The completed letter needs both your signature and the agent’s signature. Our team checks it and submits it through SelectHealth’s agent portal. We distinguish a signed request, a submitted request, and a carrier-confirmed change.
No. This page’s AOR process is for SelectHealth Individual coverage, including Marketplace coverage. If you have a different type of plan, call for help finding the appropriate next step.
No. Optional plan updates are a separate choice. They are not required to ask a question, schedule a consultation, or appoint an agent.
